Medicare costs for 2026 and 2027

Here is what Original Medicare costs this year, and what Medicare has already announced for next year, in plain words. Plan premiums are separate and vary by plan.

What we know about 2027 so far. Medicare has set the 2027 Part D limits: the most a plan can charge for its yearly deductible goes to $700, and the yearly cap on what you pay for covered drugs goes to $2,400. The 2027 Part A and Part B amounts are not out yet. CMS usually announces them in November, and we will update this page then.

Part A and Part B figures below are for 2026. Source: CMS, 2026 Medicare Parts A and B premiums and deductibles.

Part A (hospital) costs

Premium: most people pay no monthly premium for Part A, because they or a spouse worked and paid Medicare taxes for at least 10 years (40 quarters). If you have to buy Part A, the premium is $311 a month with 30 to 39 quarters of work, or $565 a month with fewer than 30.

When you stay in the hospital:

Part A cost2026 amount
Deductible for each benefit period$1,736
Hospital days 1 to 60No coinsurance after the deductible
Hospital days 61 to 90$434 a day
Lifetime reserve days (after day 90)$868 a day
Skilled nursing facility days 21 to 100$217 a day

A benefit period starts the day you go into the hospital and ends after you have been out of the hospital or skilled nursing care for 60 days in a row. You can have more than one in a year, so you can pay the deductible more than once.

Part B (doctor and outpatient) costs

  • Standard monthly premium: $202.90. Many people have it taken out of their Social Security check.
  • Yearly deductible: $283.
  • Coinsurance: after the deductible, you usually pay 20% of the Medicare-approved amount for most doctor services and outpatient care. Original Medicare has no yearly limit on this 20%.

That open-ended 20% is a big reason people add a Medicare Supplement or choose a Medicare Advantage plan, which has a yearly out-of-pocket limit.

IRMAA: higher Part B premiums for higher incomes

IRMAA stands for Income-Related Monthly Adjustment Amount. If your income is above a certain level, you pay more for Part B. Medicare uses the income on your tax return from two years before. For 2026, that is your 2024 return. The amounts below are your total monthly Part B premium.

Single filer incomeMarried filing jointly incomeYour monthly Part B premium
$109,000 or less$218,000 or less$202.90
$109,001 to $137,000$218,001 to $274,000$284.10
$137,001 to $171,000$274,001 to $342,000$405.80
$171,001 to $205,000$342,001 to $410,000$527.50
$205,001 to under $500,000$410,001 to under $750,000$649.20
$500,000 or more$750,000 or more$689.90

Married people who file separate returns have different brackets. Ask us or Social Security if that applies to you. There is also an income-based extra charge for Part D, added on top of your drug plan premium. If your income went down because of retirement, a death in the family or another life-changing event, you can ask Social Security to use a more recent year.

Part D (drug) costs

Each Part D plan sets its own premium and copays. Medicare sets two limits that apply to every plan: the highest deductible a plan can have, and a yearly cap on what you pay out of pocket for covered drugs. After you reach the cap, you pay nothing more for covered drugs for the rest of the year. Many plans have a lower deductible, or none.

Help paying for Medicare

If money is tight, there are programs that may help. If you are not sure whether you qualify, it is worth asking.

Extra Help (Part D Low Income Subsidy): this federal program helps pay Part D premiums, deductibles and copays for people with limited income and savings. You apply through Social Security, online or by phone. Some people get it on their own, for example if they already have Medicaid.

Medicare Savings Programs: these state programs can help pay your Part B premium, and some also help with deductibles and coinsurance. You apply through your state Medicaid office:

Income and savings limits change each year. We are glad to help you figure out whether to apply.

Questions people ask us

Will these costs change in 2027?

Yes. Medicare sets new amounts each year. The 2027 Part D limits are already out and shown on this page. CMS usually announces the 2027 Part A and Part B amounts in November, and we will update this page when they are released.

Why is my Part B premium higher than the standard amount?

If your income was above a certain level on your tax return from two years before, you pay an extra amount called IRMAA. See the IRMAA table. If your income has dropped because of a life change like retirement, you can ask Social Security to look again.

Do Medicare Supplement and Advantage plans have the same price everywhere?
